Visit the Oregon Screening Project website and participate in their questionnaires to check your child’s early development.

ASQ-3 Questionnaire
The Ages & Stages Questionnaires®, Third Edition (ASQ-3™) is a series of parent completed questionnaires that provide a quick check of early development in the ages of 1 month through 5 years. Results help to determine if a child’s development is on schedule or if further evaluation with a professional may be needed.

ASQ-SE Questionnaire
The Ages & Stages Questionnaires®:Social Emotional, Second Edition (ASQ:SE-2™) is a parent completed questionnaire to check a child’s early social emotional development in the ages from 1 month to 6 years. Answers help identify your child’s social-emotional strengths and where your child may need support.
